They made the reporting process deliberately long and frustrating so medical staff would give up on reporting. To report each incident is about a 45 minute process. Now imagine if you have 5 of them on one shift. That's an extra 3hrs and 45min of work on one shift on top of everything else you have to complete. Is it any wonder why things are not getting reported. I'm also pretty sure there are incentives for keeping things on the down low. your thoughts?
